3) Nurse/Technician: please fax the Congress Secretariat proof that you are a graduate of, or enrolled in, an approved programme.
4) Accompanying persons are not entitled to attend the scientific programme.

Participants may register for the Congress by the following methods:

Complete the registration form and return it with appropriate payment details to the Congress Secretariat by fax or mail.

Deadline for Early Bird Registration: 1 September 2008

Upon receipt of the completed Registration Form and full payment, the Congress Secretariat will issue a letter of confirmation by email. Please bring along this letter with you when you check-in at the Registration Counter.

Payment
Payment of registration fees must be made in Hong Kong Dollars by the following methods:

1) Cheque / Bank Draft made payable to ¡§WCVD6¡¨.
Please note that payment in the form of a cheque or bank draft must be sent together with a completed Registration form by registered mail. Only Hong Kong dollar cheques drawn on a Hong Kong bank will be accepted.
Bank drafts will not be accepted after 20 October 2008.
2) Bank Transfer to:

Cancellation Policy
Refunds for cancellation of registration will be made subject to the following deadlines and administrative charges after the Congress:

On or before 1 October 2008 Full Refund less administrative charge of HK$500
After 1 October 2008 No Refund

All cancellations must be made in writing to the Congress Secretariat (Registration) and the refund will be processed after the Congress with an acknowledgement from the Congress Secretariat. Please note that substitution is permitted as long as written notice is received before 18 October 2008.

Additional Notes:

  1. Each registrant should complete a separate Registration Form. Photocopies of Registration Form are acceptable. You are highly recommended to register online.
  2. Registrants are advised to read the Registration Information carefully before submitting their Registration.
  3. Registration Forms received without registration fees will not be processed. Please do not send cash.
  4. The Congress programme is subject to change without prior notice. In the unlikely event of cancellation of the Congress, the only liability of the Congress Organisers is to refund all the fees paid.
  5. The information provided in the Registration Form will only be used for the purpose of processing your registration and communicating with you on the matters related to the Congress. You have rights of access and correction with respect to your personal data held by us. If you wish to exercise these rights, please contact the Congress Secretariat.
  6. All inquiries, changes and cancellations should be made in writing to the Congress Secretariat.
  7. The prevailing exchange rates are approximately US$1 = HK$7.8, EUR 1 = HK$12.1 and GBP 1 = HK$15.5. Payments must be made in Hong Kong dollars.
